Formatting issues

so this question is imputted by clicking the "+" option, then selected equation, and then graphical equation. 
So it's not a copy paste issue. No student is going to take the time to figure this out. Most of my schools see it correctly, but a couple don't. We've tried changing from 150 to 125 all the way down to 70. All it does is make the jumbled mess smaller.
As you can see the first question looks right, but the rest don't.
It looks like the table column is too small or there is some sort of wrapping.

My understanding of your issue is that the first equation was rendered correctly, while the other 3 were wrapped in a single line.

After careful review, I can see the following behavior.

• Option 1 was created as a vertical list (with each inequality on its own line), which displays correctly. In Option 1 we can see every equation is separated by a new line; there is no operator that connects them in a single equation.
• In Options 2, 3, and 4, small 2-row vertical stacks (element over) were placed inline between the equations instead of standard line breaks. This causes the equations to link horizontally and split across multiple rows within the formula itself.

I hope this clarifies the given behavior. If you want to keep it separate lines like the first option, you can edit the equation and separate them with a new line (Shift + Enter).
