How to add a pdf to a form in LMS?
I'm trying to create an application form for one of our courses in the LMS. This form will be copied and used in many upcoming courses for our program. It's important that we include this pdf for registrants to review and "agree" to as part of the registration process. My team has been using a different program for registration because the forms and surveys options for the LMS won't allow us to upload a pdf file.
There must be a way of adding this file in so we can have the registration form directly within the course. Can someone teach me how?

Im not sure if this way would work, but you want students to then submit these back …correct? What if you add to the Assignment area (your PDF form). Students would have to download it, fill it out/agree and then submit it back through the Assignment tool.

@Susan.W.104 Thanks for this suggestion! Unfortunately, it's not for resubmission afterwards. It's a "Statement of Collaboration" document that they have to read and agree to as part of their course registration process.

Not sure if this might be helpful but you can create a new content topic in the course by uploading the "Statement of Collaboration" PDF form and then, set up a Release Condition to the second/next topic in the content area that would hide that topic until learners viewed the "Statement of Collaboration" topic.
You can also consider to set up the Manual Completion method for the content topic so learners can see a "Mark as Complete" button at the bottom of the content page so instructors and Admins can track completion more accurately.
