We send a fortnightly reminder to users in a particular course, who have not yet completed that course.
Is there a way users can opt out of receiving this reminder?

Unfortunately the ability for users to opt out of IA generated emails is not available, and doesn't come under the notification framework. As you are aware when it comes to the IA generated emails you have to build the business rules into the IA criteria against which a users activity or lack thereof is evaluated.
You could create a checklist in the course for users who do not want to receive the IA generated reminders - tick/untick the checklist option to receive the reminder. The status of the checklist/checklist item could then form part of the criteria for the IA.
The ability for users to opt out of IA generated emails is a good one, you should consider making a submission into the Product Idea Exchange (3rd Party Tool)
