Hi there, thanks for the reply.
I did find this page a while back (before posting my message) and I agree it is handy however I am after instructions on how to actually create forms. I will then be able to use the information on the 'About Form Templates'.

You can find the Forms tool from the Admin tools> eportfolio > Forms

another question: can i build the form so that if a topic is selected other options appear ?
for example:
radio buttons with the choice of:
Course Material
LMS
Other
if a learner selects Course Material then another set of options appear (same with LMS but different options appear etc).
hope that makes sense

Unfortunately you cannot use the form for selecting other options based on the user response. I believe this would be in the branching option that Forms does not include it is only available in the survey tool.
