Alert going to the wrong person.

Hello, we have had 2 times now where an alert for student work submission goes to a different teacher who has no connection to the class in question. We have gone through the settings and can't find any possible link. It is not consistent, only twice so far, and with different classes and teachers. Has anyone else had issues like this? Any thoughts on what could be causing it would be appreciated.
