Is there a way to determine if students actually access/read the instructor feedback comments?

There is not a way to determine if they have accessed or read the instructor comments.
Something our team suggested would be to create a discussion where students have to reflect on their feedback. This helps with creating that feedback/learning loop.

"students have to reflect on their feedback" — How does that work, Kathryn? Where do you include that in the course? What happens if students fail to do their reflections?

One way to do this is by creating a one-to-one discussion and use it as a reflection area for the student. I have added Community documentation here and you would be looking at the Enrollment Type: Groups of # and number of users being 1.
Another way could be to have a release condition on the Discussion post. The condition type would be receive feedback on submission and then you would select the assignment from the dropdown list.
You could also set up a new Discussion Forum/Topic that is specifically for feedback/reflections.
You could always have it as part of the Gradebook for reflections, or use it as a bonus points for end of term, or in a way that would make sense with your class set up.
